I would suggest HTML with PHP code Such order page is a few hours work only (if you can code)... You will only need a base HTML page (or PHP print) and a PHP, that digests the sent form data... If you do not know how to do it, I would suggest to learn to code forms or hire a coder. These "eCommerce" form generators are not really trustworthier. Just think about it, if the form creator saves the email address field and starts spamming your clients... Cheers,
