Customer survey stopped working. Need help Please!

Hi I had a guy who created a ten question survey for my site and it was working fine until I copied and pasted the codes to word document and then back to front page. I uploaded the files to my server and I must have uploaded something wrong because the form stopped working. If I fill it out and press the submit button nothing happens. Usually I get a thank you confirmation page after filling out the survey but I don’t get it any more.
I receive the emails and I can see that someone filled it out but I see only the questions not the answers.

I have the sendmail.php and that is working because I receive the email with the questions. There must be something wrong with the form itself.

I need help figuring out the problem with this form I will appreciate any suggestions very much. Thank you in advance. Laszlo.

 

Well, using Word and Frontpage was a mistake. A big one

Anyways, try placing <input type="submit" value="Submit" name="Submit"> inside the form element, that should fix it.
