Custom PHP Form

    I need a form created with the following specs:

    It will have Approximatley

    8 Fields (some optional)
    7 Drop Down Boxes. These will have Names and Point values associated with them.

    User must Choose 7 different names (no two drop downs can be the same)
    The point value must not be greater than X

    Checkbox that defaults to being checked (newsletter yes/no)

    Captacha

    3 Radio Buttons.

    1. if chosen just sends the email to me
    2. Goes to paypal to buy now/purchase option 1
    3. Goes to payal to buy/now/purchase option 2

    When all done an email should be sent to me with all the information here, and one should be sent to the person filling it out saying thank you etc...
