Buying creation of simple sport database

I have little project that I need someone to build for me.

I need someone to build a database that takes a golf score from a registered user, puts it in the database, and calculates the input from a lookup table and produces a new result.

There would be a simple input page for the registered user and maybe another page to show results if someone wants to search the database.
