Creating a workflow Incident Report Form

    I am building a website which would serve as an incident report form. What it needs to do is, when a reporting user fills the form and clicks submit, it should automatically go to a supervisor with the filled out forms and add extra fields for the supervisor to make his/ her comments. Once the supervisor fills the form and clicks submit, it goes to a Authority who would again fill in final comments. Can anyone guide me on how I should do this? I know a bit of HTML, CSS, AND PHP. Using the client side scripting language and PHP, I would like to build the above application. Also, if you can direct me to sample websites that have something like that I can look and learn too.But your help would be appreciated. Thank youEric
