Creating A "Form Submit- Pop Up Box"

How do I create a form submit pop up box for a basic website. My website is here at CashRewardsAuto.com I need this to have basic info such as Name Email Phone w/ A message on it.

Is there an easy way to create a code for this? I use Adobe Dreamweaver CS3
