Buying Create a Form with Questions I Have

Hello,

I have a questionnaire that has already been setup. The questionnaire is very similar to something like this ( FORM 1 ).

With the form I have, when I click Submit, it opens Microsoft Outlook.

I want the form to be submitted to my email address, along with all the data that someone inputs in the form.

I try the link form it does open any MS Outlook. If you want to send form data in your email ID, I can do this.

I think it is easier to use ready solution. Smth like php forms software can be used for your aims
