Could Someone Please Help Me Integrate Raw HTML From Aweber Into My Own Form?

OK I actually got a special squeeze page template with some form elements already included in the html, but right now it does nothing.

If I show you the site and aweber raw html, could somebody help me put in the necessary parts in the right places to make the form work?
