Cost saving calculator script wanted in Javascript $30

We would like a script making that will ask the user for the following information:

Number of cats owned: (A)

Amount spent on cat food per week: £ (B)
Percentage of cat food stolen by other cats: 0-100% (C)

Average number of collars lost per year: (D)
Average price of collar: Standard £5 / Magnetic £9 / Infrared £12 (E)

Average number of weeks cats boarded in a cattery per year: 1-52 (F)

Submit button

Saving is displayed number by number to build suspense, eg. "YOU SAVE £123.45 PER YEAR"

Saving to be calculated as (A*F*52)+((C/100)*(B*52))+(E*D).

The form needs to be designed nice and neat to use minimal space on the web site.
