Buying Convert two page PDF form to fully editable word

    I have a two page medical form with a considerable amount of information in multiple columns. It is currently in pdf format.

    I need someone to convert into word format. I do not care if you use a conversion tool but it needs to be editable in the same way as a word document (i.e. a conversion using pdfzilla puts every paragraph on its own layer which is not what i'm looking for)

    Type "pdf to word", "pdf to doc", "pdf 2 doc" in Google and convert it yourself. For 2 pages you'll pay $1-2 which is mere and no one will do it...
