Buying Convert to HTML a PSD landing page with email sign-up + integrate with MailChimp

I am looking for someone who can:

1. Convert to HTML this simple PSD Landing page with only an email sign-up field: http://www.epicupload.com/images/t4yfg7t67m1vl3he6o1.png

2. Integrate the email sign-up field and Submit button with MailChimp - I will give you access to MailChimp account so that you can get the code there and link to my landing page


3. Upload everything via FTP to my server
