What is the purpose of the site? A way to contact the site owner seems like a good idea in most cases. Privacy policies are probably a good idea for any site that is gathering anything approaching personally identifiable information - like what might be gathered through a contact form.

Yeah, it depends on how you are using your site. Privacy policy, about us, and terms of service pages can never hurt though. Just use a generator to create them.
Yes I have found that many affiliate programs will silently reject you if they do not see a privacy policy or in some cases a contact form.

Keep in mind that the CONTACT and PRIVACY policy pages are just as much for the protection of the website owner as well as for information of site visitors. If your policies/procedures are written; there is no questions or potential for discrepancy in the event a client has an issue and looks to file a complaint. Most times--the processes require that a client with a problem contact the service/product provider or research where they purchased for further assistance before a 3rd party will get involved to mediate. If this info is clear and accessible to clients onsite---leaves little room for interpretation or guesswork.
If I go to a site and there are no Terms of Service, privacy policy and contact us links then that is a major RED flag for me, and I leave the site.
There are some software programs that auto install these two for you. I think 90 second website builder does this.
