Contact form with option to attach a file

    Hello

    I need a php contact form with some basic fields:

    From (any name): Fake Name
    From (any email):
    To (email) :
    Subject : Anything for test
    Text content: Some info here.....bla bla bla
    Attach file : Able to attach files and send them as attachments on mail.

    Also if it is possible to not show my real server ip as sender... (I don't know if i can change it to what i want or just hide it )


    A few users can do it for free for me i know :)

    But i need to look like a real mail and keep all info in the correct fields:

    I will upload it and use it for tests only so i do not need any captcha ...

    I will test it on Linux server and it will be great if i can get this to work with as less as possible files (2 files maximum) and software requirements...

    Default installation of Centos for example it will be great...

    Not attachment of a file and not fields as i want them....
