Buying Contact form adding to simple HTML Template

    I have an HTML template I want to add a contact form to.

    The template is: http://themebot.com/free-website-templates/html-templates/5554-carbonizer

    I want to add a simple contact form into the sidebar (I will only be using this as a single page site)

    The contact form needs to take:

    Name
    Email
    Telephone
    Message
    Simple Verification (2+3)

    I then need it to send to an email address along with the domain where the message originated from.

    Please send me prices and turnaround time - I am looking for cheap and quick
