Code me a contact form for 25$

    hey i want someone to code a contact form for me.
    i have designed the form using css. you just need to code the PHP.
    and one thing more, i want an upload box included (php or ajax) too. i searched internet but didnt find any good one. the uploaded file should be attached to email when it is sent or a url to it.

    or better, please send me an email : namescombined [at] yahoo.com as i dont know why,this site doesnt open properly as my connection is very slow
    i will pay 25$ when the job is properly done. :)
