Buying Code a PHP contact form on Bootstrap template - $10 Paypal

    I would like a PHP programmer to configure a contact form for me on a responsive Bootstrap template. The contact form has already been setup, I just need to it attached to a PHP script and Ajax function to make it validate and work. The Ajax coding should generate an 'successful submission' on the same page.

    Please use your own script and not commercial one or a rip of someone else's. Open source or creative commons is fine to use. Will pay $10.
