Can't get images to appear on my optin form ?

I need help with figuring out why the images in my optin form aren't appearing on it.
I think its some sort of refrencing problem,where I have not correctly modified refrences for this optin form for it to find the images .

Both the optin form and images are uploaded to my server in the same directory.
So I think the problem is in the code on the page.

But I can't figure out where I'm going wrong ? I've tried various things but nothing seems to work ?

I think its something very simple, so I've included the code in a attachment index.html

 

Attachments do not work on DP, you might want to upload it somewhere else.

 

Can you clarify why then a facility for uploading attachments is provided, if DP doesn't work with attachments ? This is confusing ?
And when you say to upload somewhere else , where ?
