I have set up several blogs and just got a comment. When I start to approve it in Wordpress it asks me if I am sure. Seeming to say it may not be too smart. just use the plugin you get with it akismet, hasn't let any spam through. just let everything call onto akismet then if you see the odd spam comment delete it. if something says viagra its spam, or if its not related at all its spam.
When you get a comment filled with links, and which is not related to the content, you delete it and ban the IP. I thought that was supposed to be obvious
