Yes that means any links OUT from that site carry no juice.. BUT they are linked IN from sites.. so if they place a link on their site to another site they will not be penalised.. *no juice* too many do-follow links out pull your PR down.. remember PR is an algorithm of how many sites point to you regardless of what content you have on your site.

You mean the site has a PR but not in google indexed? Can you give us the site URL because if it's not indexed the site should not have PR.
First you tell me how website getting PR? it's simple answer is get quality links from high PR sites and unique content can also help so here is no connection with PR and nofollow, infact if any site is using nofollow links on his site, it will get good PR as compared to those using dofollow tag since google doesn't read their external links because of nofollow tag.
some times some sites are penalize and Google remove there back links and index but there PR will be listed on that sites and when next PR update then there PR will be also gone but if you are asking about new site which already have 0 PR and google penalize then that site can't get PR in any case
yes of course all websites with nofollow can also have page rank, the only purpose of nofollow is your website not sharing pr juice to any websites that are link to your site this strategy used to avoid spammers on your site.
It's possible. Sometimes webmasters do that on purpose for their robot.txt file so that certain page doesn't get indexed especially if the page if used exclusively for selling links.
