Yes, you should certainly keep the content and file structure in tact, that's essentially what you're paying for. You can redesign the look as much as you want. Just create a new template and copy & paste content into it. Then, override the existing files. As long as you don't change the file structure, everything should be fine and Google will re-index your site within a few weeks. You rankings will hold.
