    Create a sitemap with the new style of URLs. Be sure that old URLs give 301 redirect when crawler reaches old style pages. Then all you can do is to wait.

    Nothing easier than to get the site crawled again, but that won't help if your site is banned by Google.
    To achieve a reinclusion there is more to do.
