Best practices when changing domains

I'm in a situation where I am forced to change my domain. I'm in the process of creating a duplicate of the site on the same server using the same database. My understanding is that a 301 redirect in the root of oldsite.com will be the best solution from an SEO perspective. Following that, I'm sure it's not a bad idea to have some links pointing to the new domain.

Q: Is this the best practice? Opinions on what's in .htaccess to process the 301.

Q: What's the expected timeline for Google to get this right and expectations of rankings loss?

Q: If there are specific pages that are ranking well for key phrases, should I handle those differently?

Q: How do I handle subdomain redirection?

Q: Any suggestions or best practices to aid in the overall move?

Thanks in advance for your replies.
