Best plugin for a form

Need a form for www.propertyhogs.com/hogblog so people can fill out forms and tell us where they look for houses.

Which is the best plugin to use for that?

I'm looking for something that has a real estate form feel to it, one that you can add information on what pricerange you're looking for when buying a house etc.

 

Can't it be customized?
I don't think a "real estate form" plugin exists...
