None of the above. Don't link to "index.html" , instead like to "/" As far as setting your links up as www or /a.html it doesn't matter they all go to the same place. Rumor is it's best to use the www version but I don't believe it matters. I link all my pages relative to the site root and it works fine. BUT - don't cause your homepage to split pr by linking to it with multiple urls. _www.mysite.com and _www.mysite.com/index.html are viewed as separate pages. I did that once by accident and my homepage got a pr2 and the internal pages got pr3's.
If you have an established site then I would advise using a vertical architecture like www.example.com/directory/ as this helps build your structure. Using directories and proper page titles will also help your visitors locate where they are quickly, especially when they arrive on a deep page within your site after searching. However, if you have a new site (which I'm guessing it is) then go for a lateral and flatter structure so that all your pages are as close to root as possible. Unfortunately you may well find that Yahoo and MSN have problems spidering content deeper than the root directory. While Google will be able to spider content whether it is structured laterally or vertcally, a new site will probably be sandboxed. Therefore, my advise ... if you want it ... is to go for a flat architecture to start with in order to get as much content spidered asap. Then once you have established your site, 301 those pages to a new location at www.example.com/directory/keyword.html. Just my 2 cents.
