Not as good as they used to be, but http://www.bidvertiser.com still comes at the top of my list. I used them quite a bit in the past. The good thing about them is you can get paid via Paypal and the payout minimum is low.
How hard is it to get approved? I have several niche sites with no ad placement.... but if they are not reallly good at targetting the niche, then it will not convert well at all....

AdX isn't an alternative to AdSense because if you are banned/blacklisted on AdSense you are banned/blacklisted on AdX. I used bidvertiser years ago for personal sites - last I checked it was auto-approval for the account set up, then you have to get the URL approved.
Honestly speaking there is not a single alternative of Adsense, although you can make more money than using adsense by blogs like Copyblogger style. In short no one can replace Adsense, for best practice use one of them along with Adsense. If you have not adsense try to focus on affiliate ads, this is more productive even on low traffic blog.
I use http://www.media.net Also make it a point to use Disqus commenting system to increase your earnings. In a matter of just 6 months, I'm about to get $100 from Disqus commenting system.
