Adbrite is more than enough to cover hosting fees. Anyways if you can sign up go for adversal... They are the best...
For monetizing a proxy sites i would recommend combining Adsense with 1/2 CPM networks. See this list of CPM networks. First one pays up to 1.80$ per 1000 impressions of popups (UK/US based traffiic ) and the second one pays up to 1.00$ per 1000 impression of inpage banner ( can be placed on proxified page ). Both pays through Paypal, and the second one has very low minimum payout ( 2.50$ ) and pays every Friday.
Adversal Is great one for proxies , Got paid around $114 from them Until now and counting . Adbrite would be another choice but the earnings of it is not very good to me
