Best contact form plugin to do this with wordpress?

I want a user to be able to fill out a form that contained several input boxes such as:

Name
E-mail
Country
Product Name
Review

What's the best plugin that would allow me to configure the form in the wordpress backend and than use a shortcode to insert the form on a wordpress page.

Would this be possible using a free plugin?

 

Ya of course friend... You can use contact form 7 plugin to do this one in your site.. It is totally free..

 

Free option is Contact Form 7
If you want to take it to atmost perfection go for Gravity forms.

I decided to try Fast Secure Contact Form.

Took 2 seconds to setup with custom fields and it works.

if you want something simple Fast Secure Contact Form is very good

if you want meany options then Contact Form 7 is not the only option try cformsII

 

Contact Form 7 is definitely a good choice. My site uses that as well.
