As far as I know it can be either, but advertisers do tend to opt for the CPM option a whole lot more.
i opted out of those garbage cpm ads back in january, and lately i've been seeing a rash of image ads. they take over the whole ad block, and sometimes the same advertiser takes over all the ad blocks on the page at the same time, with repetitive image ads that all say the same thing... adsense support refuses to do anything about it. so goodby to the image ads, do yourself a favor and shut 'em off... next up for me is getting rid of the video ads, the payout on those things has declined significantly.
i feel image ads do better at times...especially if its appealing...but as danimal said..that also happens with text based ads too...
I don't think it is possible to choose just CPM ads in Adsense? or you mean you opt for CPM as an advertiser?

i think that publishers can set up cpm channels for the advertiser to target... but even with that, the publisher can't exclude any of the other pages on the site from getting the same cpm traffic... all the cpm channels do is give the advertiser an additional option. if i am correct in that, the publisher can in effect opt for cpm, on just those channels(?)

Under the AdSense for content subtab, make sure you've selected custom channels. Click the radio button next to the channel you'd like to make targetable, then click the corresponding 'edit settings' link. Click the checkbox marked Show this channel to advertisers as an ad placement and enter a description for your channel. Keep in mind that advertisers will see the name and description, so you want to provide something accurate and informative that will represent your site well. Click 'Save channel'. Keep in mind that if you have a custom channel that spans multiple sites, advertisers will see it split up into multiple custom channels by site.
