    When I completed the online form and clicked to complete i just got a blank form back on the screen. Should there have been a message to say that the form had been accepted?
    I have tried using firefox and Google Chrome but with the same result.
    I am concerned that I have either failed to submit an application or have made multiple applications.
    Any help greatly appreciated.
