Anyone know a Form creator program like this?

    The site looks pretty amazing, with customizable forms, but I'd rather not use a membership site if I can avoid it by finding some open source Ajax or PHP application that does the same.
    It is basically a drop-and drag-form for creating really customizable forms, including ordering forms. i need something open-source, free, and OSX compatible.

    Google has only delivered free "trials" and windows tools.

    Any body got a good one?

    That's just a parked domain :(

    formassembly is the best i've found, and i did A LOT of searching. they have every option i could think of.
