www.jotform.com The site looks pretty amazing, with customizable forms, but I'd rather not use a membership site if I can avoid it by finding some open source Ajax or PHP application that does the same. It is basically a drop-and drag-form for creating really customizable forms, including ordering forms. i need something open-source, free, and OSX compatible. Any body got a good one?
