I used to study spanish a while ago and thought i would try tanslating this. Tell me if i'm right.

Estaría interesado en el seguro Premiun y a fin de facilitar la propuesta les informo que nací el 14 de julio de 1940, soy empresario.

Espero que cuando pueda me haga llegar por email la propuesta de sus servicios y que cubra el seguro de salud, de accidentes y de vida.
