Any recommendations for floating footer/banner networks USA traffic?

I run a news website that gets about 2,000,000 - 3,000,000 impressions per month, right now I use adsense, facebook audience ads, ad supply, and content.ad. I also am playing with bidvertiser but am not that thrilled about their CPM right now. The site makes between 10k - 15k a month on those, and will be increasing monthly. I have tried and ruled out the following for the following reasons:

    clicksor - porn redirects/malware
    adsterra - Malvertising
UberCpm - random redirects to porn/malware
media.net - Incompetence, worst customer service in the industry
clickviper - Too low of payouts
revenuehits - injects popup codes to all your links
adcash - as of this moment, 50,000 impressions, hundreds of clicks, under $1.00 payout, extremely low earnings.
infolinks - Lol they messed up and didn't approve me for "not enough traffic" though my domain is clearly about to breach the top 100k on alexa, their loss
    chitika - extremely low payouts

What I am looking for and I'll try anything, is a banner ad network that pays something respectable, for both 728x90 and 300x250 type banner ads, and I also want a 728x90 floating footer for the bottom of the screen, ad cash and rev hits have this, but I am not satisfied with what they pay.

I run a nginx/php-fpm stack via ndeploy on a dedicated server with 2 12 core CPU's with mobile caching so I get 2 caches, one for desktop, one for tablets/phones. Traffic is roughly 80% mobile / 20% desktop, all coming from facebook. I inject ads at content top, 2nd paragraph, and the 5th paragraph, the ads I would run would be 5th paragraph and possibly 8th - 9th on mobile, and for desktop a 300x600 or 160x600 sidebar under the adsense sidebar, and possibly either 728x90's in the content between various paragraphs or the 328x266 (whatever this ad size is I don't have it exact).

Does anyone have any recommendations for banner networks that would work well with a news website that gets that kind of traffic, I don't think approval should be an issue as it gets lots of traffic.

I currently get about $6 - $8 page RPM at the moment, but I know I could squeeze maybe $1.00 more out of it if I found the right network for the ads mentioned. I do not need popunders or interstitials, I'm already getting somewhere around $15 CPM for popunders with ad supply on the desktop and am content with that.

And before someone says "Let me google that for you" trust me I did that and went about 5 pages deep and have tried probably 20 - 40 ad networks now, most of the ones I find people recommend on google either don't pay enough, redirect to porn, or malvertise and I can't have any of that going on on this website.

am on the same boat man.
    But as per my experience till date try following;

    Conversant (former ValueClick)
    CasaleMedia
    Exponent (former TribalFusion)

Above three are most reputed company and has been in business for more than a decade.
I am too searching for some good Banner Ad companies without any malware, porn redirects or popups. But seems too hard to find now in today's market.

Have you tried facebook audience? They only have 300x250 and 300x50 and you can only display them on a mobile version of your website, but they pay about the same as adsense. If you have a wordpress site you can use ad inserter to make sure they only appear on mobile/tablet and you can rotate between adsense and facebook by using |rotate| between the ads. That helps with ad blindness too since they look a bit different.

That's all I found though, smaato seemed to be paying good but for some reason they break the formatting on my site.

My 65% traffic is still desktop, so i dont think mobile ads would perform much on my website.
