Buying Another Simple Form Creation

    Hello,

    I'm looking for a form to be created for my site. It should have the standard Name/Email/Address/Billing information fields. When the field is submitted it should be sent to an e-mail adress that we provide.

    Nothing fancy, just standard form design and standard buttons.

    Which would just be more proof someone was trying to slander another member. Course it doesn't matter since it's only being counted once.

    Anyway ill do this for you for 10 bucks. Ill even make it with jquery and throw in a backend to hold all reseved messages etc along with sending you the email.
