    You are correct but make the folling edits:

    <a href="http://www.abc.com/keyword-phrase.php" title="Keyword phrase text">Keyword phrase</a>​

    This is good for all outbound links. But you should also consider the "link partner" link text as well.

    <a href="http://www.abc.com/" title="Keyword phrase text">Keyword phrase...more text</a>​

    where the title="Keyword phrase text" matches the href's page <title>Keyword phrase text</title>
