Allintext has been changed?

I was under the impression that "allintext" operator was used to rank pages based on how many times a kw phrase shows up on a page (weighted for position as well).

When I try to do allintext searches I see sites in the top 10 that have 1/2 of the instances of the kw phrase that the next 10 results have.

So what exactly is going on with the allintext?

ALSO, on a related note, when searching for a PHRASE with allintext, do I need to do it in quotes?

No need to do it in quotes. I have also noticed things being a little funky with the allintext factors. I am also wondering if new sites are not somehow being sandboxed for this string as well, but what the hell do I know LOL.

Since the most recent Google update my rankings have stayed the same, but instead of being on the first couple pages for allintext for the keywords I'm targetting, I've dropped off entirely (or, at least not within the first 200 results).

Anyone else seeing this? Any ideas?

I'm pretty sure allintext is a regular Google search, with the added constraint that pages in the results must contain all of the keywords that you searched for. Most people probably think this constraint is the default, but it's actually not.
