AJAX/PHP/MySQL - a simple example please ?

Hi,

I'm after a simple AJAX example that consists of 2 html pages (1 for writing and 1 for reading) and a MySQL database. I've been googling for a few hours and trying code that I find, but I still can't find anything that does it simply.

Can someone provide some code, or a link to a simple AJAX form that writes to a database and a simple AJAX page that reads from the database.
