Buying AJAX Hero Needed to do a Simple Form

Hi there..

Im looking for a AJAX Hero to do a nice form for me.
All you need to do is a form like this,

I will give my fields and then you have to do..

We can use some jQuery Plugin for InLine Validation also.

And main thing I need is,
AJAX Submit for the whole form in the SUBMIT button.

And when it submits via AJAX, it shows it there on that page directly a thank you message.

Also, all the field entered values are emailed to a specific email ID.
I need this done perfectly.

UPDATE : I had hired a guy from DP and he dint finish the job in 3 days also. So,im looking for more guys now..

Make sure, you have knowledge in applying jQuery readymade plugins, CSS, JAVASCRIPT, PHP and AJAX perfectly..
Or else, please dont waste my time..
I need it done in few hours now.
