AJAX Form needed using DB

    I need a AJAX page, which will have the following function:

    Ask for a password in a field
    -On exit from password field after entry, utilize AJAX to check password against DB (something like the username field on http://linkexchangetracker.com/index.php?module=authorisation&action=register)
    If no match, show error message
    -If match, show information from the matched record
    -show text area for input (make text area look like a notepad with lines)
    -on submit, send email with text area info as the body and show thank you message
