Affiliate website, is publishing own name is necessary by law?

I'm currently researching what is necessary for becoming affiliate with own website, and the main obstacle seems to be the necessity to publish own name on website, and add it to each email on Opt-in newsletter list.

What are legal requirements, US and Canada? I had read Can-SPAM and FISA.
And it seems that Amazon requires putting notice like "I, (your name), ... ". Are all big affiliate programs require the same?

Are there any services for affiliates and alike - like domain privacy is for domains - where you can register and they have all your verified information, but it'll be provided only to law enforcement and such, or like eBay, where name and address are given only during transaction.

No dba in Ont, Canada - both legal name and dba are required to be used together.

I completely agree with thought that using real name is a right thing to do, as long as it's not extremely rare (then name = address, safety issues), same as serial killer, not unpronounceable ethnic, and if you don't have daytime job that is better to keep separate.

    Thank you.
