I know this may be a little specific of a request, but I would like to get your referrals on affiliate programs that offer promotional material where you can put their promotion ad on your site that is embedded with your affiliate code, and the surfer types in their zip code or fills out a form from your site, clicks the "GO" or "SUBMIT" button, and then is forward to the next step of the process at the affiliate program's site. So far I know DentalPlans.com offers this ( example of it in live action at an affiliate site www.dental-coverage.us ), but I'd like to find other affiliate programs that offer this specific type of user-input-on-the-publishers-site type material.
