Advice required about "Supplement Stage" of a web page.

I have some enquiries. Please give me some idea about the following problems

1>Why a particular web page goes to "Supplement" stage?
2>Is there are any kind of methods or procedures to prevent this?
3>If a web page in "Supplement State", then who can I remove it from this stage?

The first thing to do is to make sure that every page has unique Title and Meta Description tags. Secondly, make sure that you have a good internal linking structure (+sitemap), and also get in some deeplinks from external websites.

This isn't a complete solution, but poor Titles, Descriptions and Links are the most common reasons for being in the Supplemental Index.
