Seems like your site has been flagged as MFA (Made for AdSense). Your main pages look ok so I doubt those were the reason. Did you have AdSense on any sub-pages with less relevant content? Google could have detected one of those (e.g. through very high CTR) and then blocked the whole domain.
I agree, try and add original content and not put too much adsense "above the fold" - see how it goes
Could be, but I would consider it unlikely. A) The email would have stated that reason specifically and B) missing a privacy policy is not something which would typically lead to a block without a warning first.
That's true. Maybe the site doesn't work in some browsers properly! make sure you check it in opera, chrome and mozilla. Just a guess i havent checked it myself
