It doesn't matter and has nothing to do with Adsense at all.

AdSense has nothing to do with your out going links. AdSense is served in iframe using Javascript. There is no link involved. But you can use nofollow for all other links on your web site. The syntax is

    <a href='websiteurl' rel="nofollow">anchor text</a>

What a funny question? You know google dont crawl adsense, do you know the reason? Google Adsense uses java script. The Outgoing links are not <a href> links. They are java scripts. No spider will crawl those links, as they are not links at all.

If you still need more explaination, just assume, people instead of making link building, they simply advertise in google, and they get 100,000 backlinks in 1 day, with high pagerank sites. :D.

Hope, you get understand, and i want to ask one more question. Where are you looking to keep rel="nofollow" ? :D
