This is my take on adsense direct. If you have millions of page impressions a day, you can negotiate with Google for AdSense direct just like MySpace did. Google gets exclusive or negotiated rights to your pages for ad purposes and you get guaranteed revenue.

Ok...so how many of us are actually getting over a million page impressions per day. Probably not that many, so Adsense direct will be for super elite websites....probably not for the masses.
You are wrong on this. There is no page impression limit/criteria to use AdSense direct. For AdSense direct you search and negotiate the pricing with the advertiser directly and charge them for searing their inventory in your adsense ad units (by replacing standard Contextual ads). Pricing is on daily basis (no CPM, CPC etc). and Yes Standard TOS is applied for publishers.
