    You have to register first with google to participate in Adsense. Then submit your site or blogs to get approval. If google adsense approves your site they will provide you with a code number. You should place the adcode in the html portion of your site where you wish to see the adsense ads.Once you are getting this approval for your site this code can be used for your other sites and blogs also. No need of separate registration for each and every site.To get succeed in this endeavor your site should have enough traffic.

    make sure you follow all Google TOS before apply AdSense account...
    also your website need very good content in order to get approve by Adsense.......create website for quality content not just for adense...
