Adding data with checkbox

Hi. I am new to website designing. I have made one form which is containing checkboxes and upon selecting the checkboxes the item should be added into the database. How to get the values from the checkbox? Upon selecting the checkbox it gives the value on but I am not getting the proper value. Please tell me the solution. Thanks in advance.
