'Bank Name' is the name of your bank.

I am not sure what it shows on yours but on mine, it shows :-

    Country :
    First Name : (Your Name)
Bank Name : (The Name of your Bank)
    Account Type :
    Institution Code :
    Branch Code :
    Account Number :

You do not need to type in all caps, just type as normal would do.

I live in Canada, and when i want to add a bank account it says:

    Country
    1. Bank Name
    2. Account type
    3. Bank Transit number
    4. Institution Number
    5. Account number

    :(

Since your name in paypal should match that of bank account they probably do not ask you for it again here.

Bank Name would be the name of your bank. Other details would relate to your bank. The Bank Transit Number would be your Routing/ABA number, if you do not know what it is, ask your bank.
