add few features in vbulletin

What I need to be added:
A. When they send donation, they will be required to place the following [text boxes]:

1. Name
2. Website Url (optional)
3. Amount (already in the mod)

B. Choose Amount [text box] (this feature is already available in this mod)

What I need is when they place an amount, there will be a text appearing where it will show how many they have bought. Example, when they place $1, an text will load showing 1000 points. When $4 is placed in amount, 4000 points will show. This is for ranges $1 to $29.

For ranges $30 and above, every $1 will show 1000 points (same as above), but a new message will appear after $29. Converting it for every $2.5 is equal to 1 day. Which means, if they place $30, 30000 points will appear, and 12 days.

What I need are the text boxes, the text appearing depending on the amount placed on the textbox, and the PM sent. Paypal functions is already ready, no need to reprogram them.

name of mod is: cyb - paypal donate 4.4

this is short work actually. -textboxes, few new sql rows, and javascript text appearing..
