    I dont live in the US. What shall I do. Thanks.

    That's just informations for USA citizens... and you should fill that fields like Daz does :)

    Note that the name you enter must be the EXACT same one you used to open a bank account in your country.

    It may seem obvious but some people might enter a shorter version of their names and when they get the check they can't clear it because the bank says particulars are different.

    Some banks are pretty strict! Happened to me :p

    Make the check payable to you and I then either leave the other 2 out or type in "international" or "intl" or n/a.
